Please read carefully. Incomplete applications will be returned to the mailing address provided.
- In the event the applicant household is married, the application must be completed by the Head of Household detailing all information for all household members (Head of Household, spouse, co-head of household, and all minors). Both adult household members (as well as any dependents 18 years of age or older) must sign the Application for Housing and HUD-9887/9887A Consent to the Release of Information.
- When the applicant household is comprised of single adult household members (co-applicants, roommates, related or unrelated adult individuals, with or without dependent children, etc) each adult (defined as 18 years of age and older or emancipated individuals not yet 18 years of age in accordance with applicable state laws), who reside in the apartment must complete and sign a separate Application for Housing and separate HUD-9887/HUD-9887A Consent to the Release Of Information.
- Please complete all questions in ink. Do not leave any section, question, or item blank, even items that you believe to not apply to you or your household (this includes income, asset and expense questions). Rather, should you know an item does not apply to you, please simply print “N/A” (not applicable) in that item. If you make a mistake and need to make a correction, simply draw a line through the erroneous information, write the correct information above, and initial the change.
- When completing the Rental/Residency History you must include all the places you and any other adult household member lived during the previous five years, starting with your current physical address. Include places where you or their name did not appear on the lease and places where you or that may have used a different name. Also note, any gaps in the five-year history, must be explained in writing at the time you apply.
For every HOUSEHOLD MEMBER, you must provide a copy of the following:

- ADULT MEMBERS- Provide copy of a valid state issued id, copy of social security card, and birth certificate.
- DEPENDENTS- Provide copy of social security card and copy of birth certificate.
Please Note:
The “pre-screening” considers all information disclosed on the application; most notably, household composition (the number of household members) to insure the property has a unit type (size) in accordance with the general occupancy guidelines, the household gross annual income meets the income restrictions for the property and program, and there are no disqualifying issues pertaining to current or prior housing and criminal history.
As part of the “pre-screening” process, we will complete Landlord Verifications. Below you will find a Landlord Verification form. Please sign and date in the Applicant’s Signature section and return with your application packet. DO NOT COMPLETE THE LOWER PORTION OF THIS FORM, only sign and date accordingly and return. We will then complete the top in accordance with your rental/residential history and reach out for these verifications.
In order to expediate processing, prior to mailing your application packet, we strongly encourage you email the documents to our office, so we can review the packet to ensure all components are completed accurately. This can be done by scan and email or simply take photos of the forms and email (photos must be clear, legible, and include the entire page within the image). Email to contactus@imcsllc.com

As long as your application remains on the waiting list and is on file with management, it is primarily the applicant’s responsibility to contact the office/manager to notify us of any changes in income, physical or mailing addresses, household members, or contact information.
If you cannot be reached by the contact information provided in your application, Integrity Management Company, will move on to the next person on the wait list.
